SUPPORT COORDINATOR

Responsibilities

  • Assist participants in successfully coordinating their support needs and in achieving outcomes of their NDIS plan;
  • provide support coordination within the agreed timeline;
  • provide advice to participants and their carers on the evolving environment of the NDIS;
  • research and identify providers and other relevant stakeholders and build strong working relationships, and
  • prepare reports, monitor expenditures and complete all associated administrative work
  • The development of complex service plans to address barriers
  • Negotiate appropriate support solutions with multiple stakeholders and seek to achieve well-coordinated plan implementation
  • Liaise with NDIA, NDIS Quality and Safeguards Commission, and other key stakeholders as required
  • Ensure all mandatory reporting is undertaken and required assessments completed within timelines and of a high quality
  • Ensure that all funding management activities are undertaken and that participants are able to maximise outcomes outlined in individual plans
  • Assessing the needs and coordinating support for NDIS participants and setting budgets
  • Identifying potential improvements to the existing services and processes
  • Evaluating and prioritising marketing opportunities
  • Conducting NDIS plan and service agreement reviews
  • Maintaining documentation of contacts
  • Ensuring adherence to company policies and procedures
  • Research, coordinate and manage a range of varying levels of support across multiple providers, to suit participant's needs

COMPETENCIES AND EXPERIENCE

  • Experience with complex case management and ability to develop strategies to address barriers and points of crisis within a multi-disciplinary team.
  • Experience implementing plans, monitoring and follow-up to negotiate appropriate support solutions with multiple stakeholders and seek to achieve well-coordinated plan implementation
  • the different types and lengths of orders a participant might need to adhere to in order to re-integrate into the community
  • understanding and respecting the participant’s privacy and balancing this with information shared with the community and mainstream providers

ACCOUNT MANAGER

Responsibilities

  • Maintaining, creating bills under the NDIS (National Disability Insurance Scheme) and being responsible for participants accounts
  • Being responsible for Reconciliation of all client accounts
  • Ensuring that all the provided information including the financial reports are accurate and compliance with statutory requirements.
  • Preparing the IAS and BAS report in a timely manner as well as superannuation payments
  • Conducting payroll tax and PAYG as well as all GST lodgements
  • Preparation of cashflow forecast in a timely manner or even as requested.
  • Preparing the income and expenditure reports for the business as well as examining the operating costs
  • Ensuring that all issued invoices to the participant are accurate
  • Conducting annual financial audit every year as per the directors’ guidance
  • Ensure the participants costs are covered in the prices that we provide

PLAN MANAGER

Responsibilities

  • Receiving, checking and uploading service provider invoices to our plan management software,
  • Preparing and uploading claims (ABA files) to the NDIS portal (PRODA),
  • Preparing supplier payments for approved claims,
  • Maintaining and monitoring NDIS plans, service agreements and funding allocations,
  • Preparing and distributing monthly plan management activity statements to NDIS participants,
  • Handling and resolving urgent matters and time-critical issues,
  • Strengthening the capacity of participants in their understanding of their budgets and plans,
  • Contributing to finance process development by identifying process improvements and efficiency gains.
  • Providing support and mentoring to junior staff, as required.
  • Ensure that all employees receive training, instruction and information to enable them to perform their work.
  • Communicate the company’s NDIS policies and processes to all employees and is also responsible for communicating risks and responsibilities for the NDIS system.

DISABILITY SUPPORT WORKER

Responsibilities

  • Support clients to participate in age-appropriate activities and tasks including employment, in-home support and community access.
  • Support clients to access activities and outings in the community.
  • Provide direct assistance to each client in accordance with their service plan and as directed
  • Work in accordance with the medication administration policy, work health and safety procedures and client rights policies including privacy and confidentiality.
  • Monitor the safety and well-being of the clients and report any concerns to the Manager.
  • Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication books as required.
  • Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
  • Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
  • In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
  • Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
  • Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.

  • Cleaning residents' rooms & bathrooms, dining and lounge rooms as well as other associated areas
  • Cleaning windows and other glass surfaces.
  • Using commercial polisher, steamer, and other associated equipment
  • Sweeping, mopping, waxing and polishing tiled, vinyl, timber and concrete floors.
  • Cleaning carpets in patient rooms
  • Scrubbing responsibilities in the kitchen
  • Vacuuming carpets, curtains, and upholstered furniture.
